Sport Options/Season Breakdown
Sports Available
- Fall: Coed Cross Country (6th/7th/8th grades), Girls Sideline Cheer (7th/8th grade), Boys Tennis (6th/7th/8th grades), Girls Volleyball (7th/8th grades)
- Winter I: Boys Basketball (7th/8th grades), Competitive Cheer (7th/8th grades), Girls Swim (6th/7th/8th grades)
- Winter II: Girls Basketball (7th/8th grades), Coed Wrestling (6th/7th/8th grades), Boys Swim (6th/7th/8th grades)
- Spring: Coed Track (7th/8th grades), Girls Tennis (6th/7th/8th grades)
Four Seasons:
DLMS 7th and 8th graders have the opportunity to participate in three sports seasons. This structure gives students more opportunities to try different sports and find their passions before entering high school.
- Fall: 8/25/26 - 10/14/26
- Winter I: 10/19/26 - 12/16/26
- Winter II: 1/04/27 - 3/03/27
- Spring: 3/15/27 - 5/25/27

Parents and families have two options for viewing DLMS athletic schedules:
Add the DLMS Athletics Calendar to Google Calendar
For the easiest access to schedules, families can subscribe to the DLMS/KMIS Athletics Calendar. Once added, games and events will appear directly in your Google Calendar.
- Copy the calendar URL below.
- Open Google Calendar on a computer.
- On the left side, find Other calendars and click the +.
- Select From URL.
- Paste the calendar URL into the URL of calendar field.
- Click Add calendar.

FinalForms
FinalForms
FinalForms is a program used by both middle and high schools for parents to register their children for sports participation or tryouts. Registration on FinalForms opens on June 1. If your child is new to middle school sports, please visit the FinalForms website to get them registered. If your child is unsure about which sport they want to participate in, you can still complete the registration and leave the sport selection blank until a decision is made. Once a sport is selected, additional forms will need to be completed, and you will start receiving emails from the Athletic Department and coaches with important information.

Sports Physicals
Sports Physicals
All Duncan Lake students participating in a school-sponsored sport must have an athletic physical on file. This physical must be completed, signed, and dated by a physician. Print the form below and return it to the DLMS main office.
School-sponsored sports that require an Athletic physical are Coed Cross Country, Coed Wrestling, Coed Track, Girls Volleyball, Sideline Cheer, Boys & Girls Swim, Boys & Girls Basketball, and Boys & Girls Tennis. All physicals are valid for one year.
IMPORTANT NOTE TO PARENTS: All student-athletes must have a current physical on file before participating in athletics. Per MHSAA requirements, the physical must be dated on or after April 15 of the previous school year to be valid for the upcoming school year.

Conference Information:
Duncan Lake Middle School (Caledonia) is a member of the West Michigan Middle School Conference (WMMSC). This conference is made up of the Central, East, Northeast, Northwest, South, and Southwest Divisions. Duncan Lake is a member of the South Division along with East Grand Rapids, East Kentwood - Valleywood MS, Thornapple Kellogg (Middleville), Wayland, and Wyoming. In some athletic events, we will be crossing over with schools from other divisions.

What is FinalForms and how do I use it? FinalForms is the program used by both the middle and high schools for parents to register their children for sports participation or tryouts. When does FinalForms registration open? FinalForms registration opens on June 1 each year.
What should I do if my child is new to middle school sports? If your child is new to middle school sports, visit the FinalForms website and complete the registration process. You can update the sport selection at a later time if needed.
Are sports physicals required for participation? Yes, sports physicals are required before your child can attend any practice or tryout for any school-sponsored sport. The physical must be completed on an MHSAA-approved form.
Where can I get the sports physical form? You can print the MHSAA-approved physical form from the DLMS website under Athletics or pick up a copy from the DLMS office. Click here to access the form.
When must sports physicals be dated to be valid? Sports physicals must be dated on or after April 15 of the previous school year to be valid for the upcoming school year. Physicals dated before that date will not be accepted.
How do I submit the completed sports physical? Completed physicals can be uploaded to FinalForms at any time after June 1. If you have any questions, you may email Taylor Kastens. For fall sports, the completed physical needs to be submitted no later than the first day of tryouts.
When do fall sports practices start? Fall sports practices will begin the week of school, immediately after the school day.
What is the athletic pay-to-play fee? The athletic pay-to-play fee is $75.00 per sport. If your child is on free and reduced lunch, this fee will be reduced or waived. Please contact Taylor Kastens for fee waivers.
Which sports do not require tryouts? Coed Cross Country, Girls Sideline Cheer, Boys and Girls Tennis, Coed Wrestling, and Boys and Girls Track are not tryout/cut sports. The pay-to-play fee payment portal on FinalForms will open after the first scheduled competition.
